A few things to know before you start

  • CHC funding is difficult to obtain. The threshold is high, and most initial applications do not succeed.
  • This screener gives an indicative result only. It cannot replace the official NHS Checklist.
  • The questions cover the 11 domains most relevant to the Checklist threshold. Answer based on a typical day — including difficult moments. (The 12th domain, Other Significant Care Needs, is assessed separately during the formal DST process.)
