Has capacity

They can authorise the request

Ask them to make the SAR directly or sign a clear written authority for you to help.

Lacks capacity + LPA

Use registered H&W LPA

Attach certified proof of authority when requesting care and medical records.

Lacks capacity + no LPA

Start with authority first

Expect pushback from record holders. LPA registration is usually the more reliable route.

Fee and timing

OPG registration is £92 per LPA. Health & Welfare and Property & Financial Affairs are separate applications. Some families qualify for fee remission or exemption.

Plan around 8 to 10 weeks before the registered document is available, and allow extra time for signature or witness corrections.

Partner disclosure

CareAdvocate may receive a referral fee if you use a partner for LPA preparation. This does not change the price you pay.

LPA preparation and registration are handled by the partner or by you directly with OPG, not by CareAdvocate.

Common questions

If your relative has capacity, they can request records themselves or authorise you. If they lack capacity, a registered Health & Welfare LPA is usually the cleanest authority route.

How long does LPA registration take?

Families should plan around an 8 to 10 week registration window, though timing can vary with OPG workload and application errors.

How much does an LPA cost?

The OPG registration fee is currently £92 per LPA, with fee remission or exemption available for some low-income applicants.
